During the Opt-Out period, eligible employees may apply to receive an annual cash payment instead of staying enrolled in a City health plan. 
To qualify, you need qualifying non-City health coverage — like a spouse’s or partner’s plan, a second employer’s plan, or a plan you bought yourself.
Review the Health Insurance Opt-Out Program FAQ on Beacon for eligibility, payment amounts, and what counts as qualifying coverage.

Before you start, have ready:
A coverage letter from your non-City health plan, dated within the last 3 months
For a family payment: your marriage certificate (if applicable) and your dependent children’s birth certificates

How to apply: Complete this form, upload your documents, and submit by Thursday, June 18 at 5:00 PM.
